Job summary

Akin is seeking a compassionate Family Resources Coordinator to support families in the Early Support for Infants and Toddlers (ESIT) program in Washington. You will guide caregivers through services, rights, and options to help their child thrive during the critical early years.

You will coordinate IFSPs, connect families with resources, and collaborate with clinicians and providers to ensure timely, coordinated support while maintaining confidentiality and privacy.

Qualifications

• At least one year of experience in social services, early intervention, or a related field.
• Ability to maintain current CPR, First Aid/AED, and HIV/Bloodborne Pathogens certifications.
• A valid driver's license and a favorable driving record (no more than three violations in three years).

Responsibilities

• Partner with families to understand goals and develop IFSP goals.
• Coordinate development, scheduling, and reviews of Individualized Family Service Plans (IFSP).
• Connect families with community resources and navigate Medicaid-related resources when applicable.
• Maintain confidential case records and documentation.
• Collaborate with a multidisciplinary team and participate in case reviews.
