Job summary

Akin is seeking a Family Resources Coordinator to support families in the ESIT program. You will work with caregivers to identify developmental needs, set IFSP goals, and connect families with community resources.

This role emphasizes building relationships, advocacy, and coordinated service delivery. Responsibilities include guiding families through ESIT, maintaining sensitive case notes, and collaborating with a multidisciplinary team to ensure timely support and access to benefits.

Qualifications

• Minimum one year of experience in social services or early intervention.
• Current CPR/First Aid/AED certifications required.
• Valid driver's license with clean driving record.

Responsibilities

• Build trusting relationships with families and caregivers and identify child goals.
• Develop and monitor IFSP goals with families and coordinate services.
• Guide families through ESIT, explain services, rights, and safeguards.
• Maintain monthly communication and help families navigate complex systems.
• Coordinate referrals and collaborate with service providers and partners.
