Job summary

Akin Family Resource Centers is hiring a FRC Family Navigator to serve diverse communities, assess family needs, and connect families to appropriate Akin programs and local resources. This role emphasizes respectful intake, goal planning, and ongoing collaboration with community partners to promote family stability and equity.

The position involves coordinating across programs, maintaining records, and participating in statewide learning to improve service delivery.

Qualifications

• 1 year experience in community and/or family service.
• One-step Mantoux TB skin test prior to start of employment, unless proof of negative test within past 12 months.

Responsibilities

• Provide intake and screens to determine eligibility for internal programs and identify families' needs.
• Develop plans with families to meet needs and promote self-sufficiency.
• Provide family navigation, information, and referral.
• Coordinate with Akin programs and community resources to connect families to relevant services.
• Attend community meetings and participate in outreach to connect families to programs.
• Maintain records of referrals and navigation to track outcomes and support information sharing.
• Assist with grant and monthly reports; keep files organized.
