Job summary

Akin Family Resource Centers is seeking a FRC Family Navigator to serve as the central point of contact for community members. You will assess and navigate to connect families with Akin programs and resources, following the Pathways Framework to support family well-being and equity.

Key duties include intake screening, goal planning, referrals, coordinating with partners, and reporting. This role requires strong communication, organization, and ability to handle sensitive information across

• Excellent written and oral communication skills.

Responsibilities

• Provides intake and screens to determine eligibility for internal programs and identifies strengths and service needs for families within the community.
• Develops plans with families to meet needs and assists with goal planning for self-sufficiency.
• Provides family navigation, information, and referral.
• Coordinates with programs and community providers to connect families to resources.
• Attends community meetings and creates information sharing opportunities.
• Designs family outreach plans and participates in outreach efforts.
• Maintains referral records and tracks outcomes to support information exchange.
• Contributes to shared learning with other family navigators statewide.
• Assists with grant and monthly reports; maintains files and sends progress reports.
• Supports the implementation of FRC mobile.
• Performs other duties as assigned by supervisor.
