Job summary

The Centers in Cleveland seeks a liaison to connect families with services through the Heights Suburban Collaborative program, building trusted relationships with families, staff and partner agencies while linking people to community resources and supports that address identified needs.

This role requires flexibility to work evenings or weekends, a valid Ohio driver’s license, and the ability to identify suspected abuse for mandated reporting.

Qualifications

  • Associate Degree in Human Services required.
  • Bachelor’s degree in Human Service preferred.
  • Must be available to work flexible hours including evenings and weekends.
  • Must have knowledge of growth stages and family dynamics.
  • Must have a valid driver’s license in the State of Ohio.
  • Must be able to identify suspected child abuse or neglect and report as a mandated reporter.
  • No record of child abuse or moral turpitude per background check.

Responsibilities

  • Link families and individuals to The Centers Integrated service model and community resources; provide linkages or referrals as needed.
  • Provide front desk and reception duties to support the Heights Suburban Collaborative program.
  • Support Foster Care Cluster as mandated in the contract deliverable.
  • Maintain and organize all supplies and program-related purchases.
  • Coordinate all aspects of the Family and Children First Council Referral and Application process.
  • Maintain coordination for Monthly Collaborative meetings as mandated through the contract deliverable.
  • Build cooperative relationships with local agencies, businesses, and community organizations.
  • Maintain current Community Partnership agreements.
  • Identify and reach out to Foster or Adoptive Parents to aid in licensure completion.
