Delaware County is seeking a Family Recovery Coordinator to support multi-need children and families. Under the FCFC Administrator, you will act as liaison and facilitator to incorporate family and agency goals, helping create a community vision for improved services.

This grant-funded two-year role involves Council management support and FCSS case management with a focus on collaboration and accountability. Driving qualifications and county employment requirements apply.

Qualifikationen

• Bachelor’s degree in Social Work or related field required.
• Minimum of 1-year experience in social work, working with families affected by substance abuse, or related field preferred.
• Must meet and maintain qualifications for driving on county business as a continued condition of employment. All required licenses and certificates must be maintained as a condition of continued employment.

Aufgaben

• Council Management Support: assist in planning, development, evaluation, organization, and collaboration of Delaware County Family & Children First Council.
• Provide support for Council meetings, including maintaining minutes and records.
• Participate in Council committees and implement Council goals.
• Collaborate with Delaware County organizations to assess needs and plan funding, services, and community involvement.
• Represent Delaware County at local and state meetings or on councils or boards relating to children and families.
• Assist with creating service provider contracts for FCSS clients.
• Service Coordination: review referrals, schedule interviews, assess family needs, and create family-centered plans with clear end dates and measurable goals.
• Coordinate services with Delaware County agencies for effective outcomes.
• Maintain case records and monitor progress, reporting to involved agencies.
• Create Service Coordination meetings with agencies as needed; act as key contact.
• Refer clients to community resources as needed.
• Other Essential Functions: attend trainings, perform typing/computer work, work overtime as required, and other duties as assigned.
