Job summary

Access Foster Care and Adoption in the Delaware Valley seeks a full-time Foster Care Coordinator to support children in foster care provider homes. The role includes visiting families, coordinating services, and ensuring safety, with mileage reimbursement and regional travel.

Based in Fort Washington for weekly meetings, the role requires driving to sites across Bucks, Montgomery, Delaware, and Chester counties and collaborating with treatment teams to implement service plans.

Qualifications

• Bachelor's degree in human services, social work, psychology, education or criminal justice is required.
• Master's degree in a related field is preferred.
• Two years of direct experience with children/adolescents and/or case management is preferred.

Responsibilities

• Develop, implement and review individual service plans with the team based on strengths and needs.
• Coordinate the implementation of supports in each child’s plan.
• Collect and prepare program or individual data as requested.
