Job summary

Catholic Charities of Baltimore is seeking a Family Care Coordinator to support a child and family dealing with significant functional impairment across school, family, and community domains. The FCC will develop and implement a family-centered care plan and may provide group-based intervention to youth.

Qualifications

• High School/GED with 2 years of work experience with emotionally disturbed children and families.
• Bachelor’s degree in social work, psychology, or related field with 1 year of work experience with emotionally disturbed children and families.

Responsibilities

• Conducts a family needs assessment and develops a care plan or IRP with the family.
• Uses motivational interviewing with youth and family to pursue goals.
• Coordinates meetings and communicates with outside providers, especially schools.
• Assists families in accessing local supports including basic needs, medical, and transportation.
• Maintains written records of assessments and meetings, including EMR documentation.
• Provides updates on family progress and quarterly reporting.
• Performs other duties as assigned.
