Job summary

DCCCA Day Reporting Center seeks a Family Preservation Services Case Manager to support families and children during the referral period. The role emphasizes safety, stability, and well-being through strengths-based case management aligned with evidence-based practices.

Qualifications

• Bachelor's degree in human services is required.
• Prior work in behavioral health or children/family services is required; personal experience with addictions treatment is preferred.
• Must possess strong verbal and written communication skills.
• Demonstrates flexibility and adaptability; able to shift styles to fit the needs of a wide range of cultures, people, and organizations.
• Solid commitment to the principles of support and advocacy for foster and adoptive families.
• Must possess a valid state Driver's License, in force automobile liability insurance, and maintain a Motor Vehicle Record (MVR) that meets organization's insurability standard as defined by DCCCA policy. Availability of reliable transportation is required for this position.

Responsibilities

• Provides for the safety, stability and well-being of children and families during the referral period.
• Delivers culturally competent, strengths-based case management services to children and families consistent with evidence-based practice models.
• Decisions are made on guidelines provided by the supervisor, family preservation standard procedures and policies, evidence-based practice model and DCCCA organizational policies.
• Meets timelines to connect with the family within 24 hours of referral; facilitate initial family meeting within 48 hours; complete assessment-based case plan within 20 days; and case closure within 3-6 months.
• Engages and works in partnership with families to complete assessments and develop case plan goals and objectives.
• Meets with the family in the home weekly and monitors progress; meets monthly with each child to assess safety.
• Assesses each child's safety with every family contact and reports all suspected child abuse/neglect.
• Provides weekly home-based family skills training on topics like nutrition, hygiene, safety, parenting, and problem solving.
• Works closely with the Family Support Worker to provide services and make decisions about child well-being.
• Completes and analyzes initial and ongoing family assessments related to safety, stability and well-being.
• Develops case plans in partnership and makes community referrals as needed.
• Completes and submits timely court reports; appears and testifies in court as required.
• Collects drug screens as necessary.
• Documents case information, including court reports, emails, and activities weekly.
• Responds promptly to family crisis situations; available 24/7 to clients.
• May be required to drive to multiple DCCCA locations and other sites during the day or night.
• Promotes teamwork and maintains professional relationships with families and community partners.

Education

Bachelor's degree in human services
