Responsibilities

  • Provides for the safety, stability and well-being of children and families during the referral period
  • Delivers culturally competent, strengths-based case management services to children and families
  • Makes decisions guided by supervisor, procedures, practice models and policies
  • Meets timelines to connect with families within 24 hours of referral
  • Facilitates initial family meetings within 48 hours
  • Completes assessment-based case plans within 20 days of referral
  • Closed cases within 3–6 months of referral
  • Engages families to complete assessments and develop case plan goals
  • Meets with the family in the home weekly to monitor progress
  • Meets monthly with each child to assess safety
  • Assesses child safety with every contact and reports suspected abuse/neglect
  • Monitors progress using assessment tools and updates case plans
  • Provides weekly home-based family skills training (nutrition, hygiene, safety, parenting, etc.)
  • Works with Family Support Worker to coordinate services for the family
  • Prioritizes problems, sets goals, and uses resources to resolve issues
  • Completes and analyzes initial and ongoing family assessments including mental health, education, and relationships
  • Uses assessments to develop the case plan with Family Support Worker and family
  • Makes community referrals as needed
  • Completes and submits timely court reports and may testify in court
  • Collects drug screens as necessary
  • Documents case information weekly
  • Responds to family crises 24/7
  • May drive to multiple DCCCA locations during the day/night
  • Promotes teamwork and maintains professional relationships with families and partners
  • Engages in relevant professional development

Essential Functions
  • Provides for the safety, stability and well-being of children and families during the referral period
  • Delivers culturally competent, strengths-based case management services to children and families consistent with the evidence-based practice models
  • Decisions are made on guidelines provided by the supervisor, family preservation standard procedures and policies, evidence-based practice model and DCCCA organizational policies
  • Meets DCF timelines to connect with the family within 24 hours of referral; facilitate initial family meeting within 48 hours of referral; complete assessment-based case plan within 20 days of referral; and case closure within 3- 6 months of referral
  • Engages and works in partnership with families to complete assessments and develop case plan goals and objectives.
  • Meets with the family in the home weekly, monitoring the family's progress towards their case plan goals; and meets monthly with each child individually to assess child's safety
  • Assesses each child's safety with every family contact and reports all suspected child abuse / neglect. Monitors family progress and service provision using assessment tools, and updates family case plan based on those assessments
  • Provides weekly home-based family skills training which may include: Nutrition, Hygiene, Household and Time Management, Home and Child Safety, Basic Parenting, Transportation, Problem Solving and Communication to assist parents in providing a safe, nurturing environment for children in their care
  • Works closely with the Family Support Worker to provide services for the family and make decisions about child well-being
  • Prioritizes identified problems that require resolution, establishes goals and objectives, utilizes available resources, and arrives at decisions appropriate to the family
  • Completes and analyzes initial and on-going family assessments related to safety, stability and well-being, including mental health, substance abuse, education, emotional, social and physical well-being, parenting and parent-child relationships
  • Utilize assessments to develop the case plan in partnership with the Family Support worker/Family Mentor, child/ren and family; make community referrals, as necessary
  • Completes and submits timely court reports; Appears and testifies in court as required
  • Collects drug screens as necessary
  • Documents case information, including court reports, emails, and activities weekly
  • Responds promptly to family crisis situations, documents incidents and is available 24/7 to clients
  • Job may require the employee to be able to regularly drive to various DCCCA locations and other business locations throughout the day or night as required
  • Promotes and practices teamwork in all activities
  • Fosters and maintains professional relationships with families, community partners and peers
  • Participates in professional development that is relevant
