Job summary

DCCCA in Mission, KS is seeking a case manager to ensure the safety, stability, and well-being of children and families during referrals. You will deliver culturally competent, strengths-based services and coordinate with supervisors and community partners.

Qualifications

  • Bachelor's degree in human services required.
  • Prior work in behavioral health or children/family services required; addictions experience preferred.
  • Strong verbal and written communication skills.
  • Must have valid driver's license and reliable transportation.

  • Provide safety, stability and well-being support for children and families during referrals.
  • Deliver culturally competent, strengths-based case management per models.
  • Meet DCF timelines: contact within 24 hours, family meeting within 48 hours.
  • Develop and monitor family case plans with weekly home visits.
  • Complete and file timely court reports and, when required, testify in court.
  • Arrange community referrals and coordinate with Family Support Worker.

Essential Functions
  • Provides for the safety, stability and well-being of children and families during the referral period
  • Delivers culturally competent, strengths-based case management services to children and families consistent with the evidence-based practice models
  • Decisions are made on guidelines provided by the supervisor, family preservation standard procedures and policies, evidence-based practice model and DCCCA organizational policies
  • Meets DCF timelines to connect with the family within 24 hours of referral; facilitate initial family meeting within 48 hours of referral; complete assessment-based case plan within 20 days of referral; and case closure within 3- 6 months of referral
  • Engages and works in partnership with families to complete assessments and develop case plan goals and objectives.
  • Meets with the family in the home weekly, monitoring the family's progress towards their case plan goals; and meets monthly with each child individually to assess child's safety
  • Assesses each child's safety with every family contact and reports all suspected child abuse / neglect. Monitors family progress and service provision using assessment tools, and updates family case plan based on those assessments
  • Provides weekly home-based family skills training which may include: Nutrition, Hygiene, Household and Time Management, Home and Child Safety, Basic Parenting, Transportation, Problem Solving and Communication to assist parents in providing a safe, nurturing environment for children in their care
  • Works closely with the Family Support Worker to provide services for the family and make decisions about child well-being
  • Prioritizes identified problems that require resolution, establishes goals and objectives, utilizes available resources, and arrives at decisions appropriate to the family
  • Completes and analyzes initial and on-going family assessments related to safety, stability and well-being, including mental health, substance abuse, education, emotional, social and physical well-being, parenting and parent-child relationships
  • Utilize assessments to develop the case plan in partnership with the Family Support worker/Family Mentor, child/ren and family; make community referrals, as necessary
  • Completes and submits timely court reports; Appears and testifies in court as required
  • Collects drug screens as necessary
  • Documents case information, including court reports, emails, and activities weekly
  • Responds promptly to family crisis situations, documents incidents and is available 24/7 to clients
  • Job may require the employee to be able to regularly drive to various DCCCA locations and other business locations throughout the day or night as required
  • Promotes and practices teamwork in all activities
  • Fosters and maintains professional relationships with families, community partners and peers
  • Participates in professional development that is relevant
Education, Experience, And Licensure
  • Bachelor's degree in human services is required
  • Prior work in behavioral health or children/family services is required; personal experience with addictions treatment is preferred
  • Must possess strong verbal and written communication skills
  • Demonstrates flexibility and adaptability; able to shift styles to fit the needs of a wide range of cultures, people, and organizations
  • Solid commitment to the principles of support and advocacy for foster and adoptive families
  • Must possess a valid state Driver's License, in force automobile liability insurance, and maintain a Motor Vehicle Record (MVR) that meets organization's insurability standard as defined by DCCCA policy. Availability of reliable transportation is required for this position
