Job summary

Northeast Family Services is seeking a Case Manager to support in-home services for children and families, reporting to the Clinical Supervisor. You will liaise with the treatment team, families, and DCYF, developing plans with measurable goals and ensuring proper documentation.

Responsibilities include managing caseloads, coordinating with DCYF and schools, reporting to the court, and providing parent education and supervision in home and community settings; requires flexible, evening

Qualifications

• Bachelor’s degree in social work, counseling, psychology or related field.
• Two years minimum experience with children, youth and families in home/community setting.
• Valid driver’s license with reliable vehicle.
• Authorized to work in the U.S.; no visa sponsorship.

Responsibilities

• Maintains assigned caseload and delivers in-home services per DCYF referrals.
• Collaborates with DCYF and treatment team; documents progress and plans.
• Develops measurable treatment goals with families and DCYF.
• Completes assessment and report writing; adheres to documentation requirements.
• Reports progress to court as requested; participates in meetings.
• Provides parent education, supervised visitation, and coaching.
• Ensures child safety and interfaces with schools and providers.
