Job summary

DCCCA is seeking a Full-Time position focused on safeguarding children and supporting families during the referral period. The role involves home visits and delivering culturally competent, strengths-based services to meet safety and well-being goals.

The candidate will connect families within 24 hours, arrange initial meetings within 48 hours, and develop assessment-based plans within 20 days while documenting progress weekly and coordinating with the Case Manager.

Qualifications

• High school diploma or equivalent is required.
• Requires a minimum of two years relevant experience in behavioral health or children/family services.
• Valid driver's license with a good driving record.
• Must pass pre-employment background checks, fingerprinting, reference checks and drug screening.

Responsibilities

• Provides for the safety, stability and well-being of children and families during the referral period.
• Delivers culturally competent services using a strengths-based approach to children and families consistent with evidence-based practice models.
• Meets DCF timelines to connect with the family within 24 hours of referral; facilitates initial family meeting within 48 hours of referral; completes assessment-based case plan within 20 days of referral; closes case within 3–6 months of referral.
• Provides support and resources to family in Nutrition, Hygiene, Household and Time Management, Home and Child Safety, Basic Parenting, Transportation, Problem Solving and Communication.
• Meets with family in the home weekly to monitor progress towards case plan goals and assess each child's safety.
• Remains current on community resources and assists families in obtaining assistance.
• Works closely with the Case Manager to provide services for the family and make decisions about child well-being.
• Documents case activities weekly.
