Job summary

• Delivers strengths-based case management to children and families.
• Decisions follow supervisor guidelines and organizational policies.
• Connect with the family within 24 hours of referral.
• Facilitate initial family meeting within 48 hours.
• Complete assessment-based case plan within 20 days.
• Close case within 3–6 months of referral.
• Engages families to complete assessments and set goals.
• Conducts weekly home visits and monthly child check-ins.
• Assesses child safety at each contact and reports concerns.
• Provides weekly home-based family skills training.
• Collaborates with Family Support Worker to plan services.
• Prioritizes problems, sets goals, uses resources.
• Uses assessments to develop the case plan with families.
• Completes and submits court reports and testifies as required.
