Job summary

The Salvation Army USA Western Territory is seeking a Pathway of Hope Case Manager in Caldwell, Idaho. This full-time, non-exempt role provides intensive case management to families, helping them access housing, work, and supportive services while building individualized action plans.

You will conduct client intake, assessments, and weekly progress reviews, maintaining HMIS/Wel lSky data and coordinating with community resources. A driver’s license and dedication to service are essential.

Qualifications

• Bachelor of Arts in Social Work preferred or related field; assoc. degree with 1–3 years case management experience acceptable.
• Valid Idaho driver’s license.
• 1–3 years experience with low-income or homeless individuals/families.
• Able to pass criminal background checks and comply with Protect the Mission policies.

Responsibilities

• Qualify and determine client eligibility for POH services and financial assistance.
• Maintain a caseload of 10–15 families and conduct intake assessments.
• Provide one-on-one goal setting and action planning with clients.
• Assist clients in accessing housing, employment, and educational resources.
