Job summary

Yoh's Primary Care in Windsor, WI is seeking a Nurse Practitioner for a contract opportunity. The NP will perform physical examinations, order and interpret tests, and develop comprehensive care plans in collaboration with physicians.

Strong clinical knowledge in primary care and occupational health, excellent communication, and patient-focused service are essential. BLS, MSN, NP license, and ANCC/AANP certification required.

Qualifications

• Active NP license to practice.
• Current national NP certification (ANCC or AANP) required.
• Master’s Degree in Science of Nursing or equivalent.
• BLS certification should be current.
• Urgent/primary care or occupational health experience preferred.

Responsibilities

• Performs physical examinations and preventive care.
• Orders, interprets, and evaluates diagnostic tests.
• Documents findings and plans care and prognosis.
• Collaborates with physician and health professionals to plan care.
• Prescribes medications or therapies as appropriate.
• Refer patients to specialists when needed.
• Maintains medical records using EMR.
• Educates nursing staff on best practices.
• May take on additional duties as assigned.

Nurse Practitioner

Needed for a contract opportunity with Yoh's Primary Care located in Windsor, WI. This clinic provides care to patients ages 2+. Clinical knowledge and experience in primary care, acute care, and occupational health are expected with an ability to provide comprehensive, patient-centered care across a variety of healthcare needs. Strong communication, teamwork, and clinical decision-making skills as well as exceptional customer service is a top priority, with an emphasis on creating a welcoming, respectful, and positive patient experience.

Schedule

Mon, Tues 6:30a-4:30p, Wed, Thurs 11a-7p, Fri 8a-12p

Duration

1/4/27-5/5/27 with training in mid December (PTO coverage)

Pay

$55-70/hr DOE

Estimated Min Rate: $55.00

Estimated Max Rate: $70.00

What You’ll Be Doing

• Performs physical examinations and preventive health measures within prescribed guidelines and instructions of Physician.
• Orders, interprets, and evaluates diagnostic tests to identify and assess patients’ clinical problems and health care needs.
• Records physical findings, and formulates plan and prognosis, based on patient’s condition. Provides written home-going instructions.
• Collaborates with Physician and other health professionals to prepare comprehensive patient care plan as necessary.
• Prescribes or recommends medications or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures.
• Refers patients to Physician for consultation or to specialized health resources for treatment.
• Maintains medical records using (In some health centers it may be using Electronic Medical Record software).
• Educates and coaches nursing staff on best nursing practices.
• May require other duties as assigned.

What You Need to Bring to the Table

• All nurse practitioner licenses/certificates to practice
• Current national certification (ANCC or AANP required)
• Master Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required)
• BLS certification
• Some experience in urgent care, primary care, emergency, occupational health or related is required
