Children’s Services of Roxbury is seeking a Stabilization Case Manager to assist families in shelter as they move toward permanent housing and stability. The role focuses on assessing needs, creating service plans, and supporting families to achieve improved well-being and housing outcomes.

The position requires monthly home visits across Boston, coordination with landlords, access to community resources, and teamwork with CSR to boost financial independence.

Qualifications

• Experience working with formerly homeless or low-income families.
• Fluent written and spoken English.
• Reliable transportation for home visits and meetings.
• Proficient with Microsoft Office Suite.
• Bachelor’s Degree in human services or related field preferred.
• Spanish and/or Haitian Creole language skills preferred.
• Experience entering data into ETO or Apricot preferred.
• Must be able to climb 4 flights of stairs.
• Knowledge of community resources for family stability.
• Strong boundaries and positive relationship skills.

Responsabilités

• Meet with families monthly in their homes across Boston and refer to resources.
• Complete stabilization assessment and establish a service plan addressing barriers.
• Assist with housing stability by coordinating with landlords and providers.
• Provide housing counseling and information to prevent loss of housing.
• Coordinate legal services and attend housing court when needed.
• Support families to increase economic self-sufficiency with CSR team.
• Help families obtain health insurance and primary care services.
• Maintain client files and ensure HUD reporting requirements are met.
• Travel to local meetings and work with shelter staff.
