Responsibilities

• Provide intensive short-term case management and service coordination tailored to families’ housing needs.
• Coordinate with program leadership and partner systems to ensure quality and progress toward goals.
• Develop stabilization plans with timelines, responsibilities, and prioritized interventions.
• Maintain accurate case files and generate reports on caseload activity and outcomes.
• Assist families in accessing housing resources, benefits, and eligibility processes.
