Job summary

Pine Street Inn is seeking a Housing Navigation Case Manager in Boston, MA. You will work with a team of case managers to develop quick and sustainable housing plans for homeless guests, engaging them to remove barriers to housing and create individualized plans with supports.

The role emphasizes accurate HMIS data entry, interdepartmental collaboration, and delivering exceptional service to guests and partners to advance the mission of eliminating homelessness.

Qualifications

• Two years of experience delivering services to a challenged population, often with addiction and mental health concerns.
• Strong computer and math skills, including MS Word and Excel.
• Experience creating housing plans and individual service plans with homeless individuals.
• Experience accessing housing opportunities and subsidy programs for homeless individuals.

Responsibilities

• Participate in a team of case managers to develop and execute housing plans.
• Engage with homeless guests to reduce barriers to housing and create rapid, sustainable housing plans.
• Enter guest data accurately into HMIS within tight timeframes.
• Provide excellent customer service and coordinate with PSI departments for integrated service delivery.
