Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by developing treatment plans, delivering therapeutic interventions, and linking them to needed services across our programs.

You will collaborate with the treatment team, participate in weekly meetings, recruit referrals, maintain compliant documentation, and adhere to COA standards while advancing professional development within a values-driven environment.

Qualifications

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and the Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions, as well as skill-building, outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with specific services and resources as identified in the treatment plan.
• Serve as a liaison with other professionals, agencies, and community resources related to the client, ensuring needs are met comprehensively.
• Maintain accurate and timely documentation of all client interactions, treatment plans, and progress notes in compliance with organizational policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with all state regulatory bodies and COA standards.
• Participate in ongoing training and professional development activities as assigned, to stay current with best practices for quality service delivery.
