The role emphasizes compliance with COA standards and active referral development.

Responsibilities include coordinating care, participating in weekly team meetings, maintaining thorough documentation, and providing 24/7 on-call support as needed.

Qualifications

• Bachelor's degree in a human services field required.
• Minimum of two years experience working with children and/or families.
• Experience with developing treatment plans and coordinating care is preferred.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team.
• Provide individual, group, and family therapeutic interventions per the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with services and resources identified in the plan.
• Liaise with professionals, agencies, and community resources to meet client needs.
• Maintain accurate documentation of client interactions, plans, and progress notes per policy.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients when required.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training to stay current with best practices.

Education
