Job summary

Health Connect America, Inc. is seeking a Family Development Specialist to support clients and families by creating and implementing treatment plans and delivering therapeutic interventions in our care teams. You will link clients to services, document progress, and recruit referrals to sustain caseloads.

The role emphasizes collaboration with treatment teams, adherence to COA standards, and ongoing professional development in a fast-paced, community-based setting.

Qualifications

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and the Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions, plus skill-building as outlined in the Treatment Plan.
• Attend and participate in weekly Treatment Team meetings.
• Link clients and families with services and resources identified in the treatment plan.
• Serve as liaison with professionals, agencies, and community resources to meet client needs.
• Maintain accurate documentation of client interactions and progress notes per policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training to stay current with best practices.
