Lakes Regional in Sulphur Springs, TX, seeks a highly trained Wraparound Coordinator to lead and coordinate the wraparound planning process for children, youth, and families with complex needs. You will facilitate structured, family-centered team meetings and ensure services and supports are organized in a collaborative, strengths-based manner.

Qualifications

• Minimum of a Bachelor's degree from an accredited college or university with a degree in Health Science, Social Work, Psychology or related field.
• Must have a valid driver’s license and an acceptable driving record, as well as personal automobile liability insurance as required by the State of Texas.

Responsibilities

• Lead and coordinate the Wraparound planning process for children, youth, and families with complex needs.
• Facilitate structured, family-centered team meetings and ensure services are organized in a collaborative, strengths-based manner.
• Bring together family members, natural supports, service providers, and community partners to build a shared understanding of the family's situation and develop a coordinated plan of care.
• Empower families to have their voice and goals guide planning and decision-making.
