Job summary

Health Connect America is seeking an experienced Family Centered Treatment (FCT) Therapist to deliver evidence-based services in homes and communities across North Carolina. You’ll work with children, adolescents, and their families, using the FCT model to promote lasting change and stability.

The role emphasizes hands-on, relationship-driven clinical practice, with flexible scheduling, 24/7 on-call coverage, and opportunities to advance toward FCT Certification with organizational support.

Qualifications

• Master’s Degree in Counseling, Social Work, or related field.
• Provisional/associate license (LCSWA, LCMHCA, LMFTA).
• Qualified Professional (QP) with experience serving children and adolescents with a primary mental health diagnosis.
• Current FCT Certification is a plus, though not required — we'll help you get there.

Responsibilities

• Deliver Family Centered Treatment services in home and community settings, meeting families where they are.
• Guide children and families through the four phases of the FCT model to support lasting change.
• Develop and maintain individualized Service Plans in collaboration with the Treatment Team.
• Conduct assessments and provide therapeutic interventions and psychoeducation aligned with treatment goals.
• Coordinate care with medical, behavioral health, school, and community partners to ensure integrated support.
• Support families in setting goals, building skills, and accessing community resources.
• Develop discharge and aftercare plans to promote long-term stability and success.
• Maintain timely, accurate clinical documentation in the Electronic Health Record and participate in quality assurance activities.

Education

Master’s Degree in Counseling, Social Work, or related field
