Health Connect America, Inc. is seeking a Family Centered Treatment Specialist to provide evidence-informed services to children, adolescents, and families in home, community, and office settings.

The role emphasizes family engagement, service planning, and coordinated care across treatment teams. Requirements vary by state, but a Bachelor's degree in human services or related field is essential, with FCT certification preferred.

• Current Family Centered Treatment Certification preferred in all areas.

Responsabilidades

• Provide Family Centered Treatment (FCT) services to children, adolescents and families in home, community, or office.
• Maintain Service Plans in coordination with the Treatment Team and document in the Electronic Health Record.
• Deliver 24/7 on-call coverage to a caseload of families and children.
• Coordinate discharge plans and aftercare needs with community resources.
• Consult with medical and non-medical providers to ensure integrated care.
• Monitor progress and participate in QA/QI activities.

Formação

Bachelor’s degree in human services or related field

PCCSS credential in Mississippi (provisional)

QMHP eligibility/credential (Virginia)

LSW preferred (Ohio)

QMHP eligibility/credential (North Carolina)
