Job summary

Wisconsin Community Services is seeking a Support and Service Coordinator (SSC) for the Children’s Long Term Support (CLTS) program in Milwaukee. Qualifications

• Knowledge of or experience with children, youth, or young adults with disabilities.
• Compassionate, patient, and nonjudgmental approach to working with diverse families and trauma.
• Ability to remain calm, professional, and solution-focused during challenging situations.
• Strong communication, interpersonal, and relationship-building skills.
• Advocating effectively while respecting youth and family self-determination and culture.
• Strong organizational and time-management skills.
• Ability to work independently and with a multidisciplinary team.
• Follow written and verbal instructions, policies, and program requirements.
• Basic computer skills and proficiency with Microsoft Word and Excel.
• Sensitivity and respect for diverse cultural, ethnic, socioeconomic, and disability-related experiences.

Responsibilities

• Develop trusting, professional relationships with children, youth, families, and their support networks.
• Provide family-centered, trauma-informed, strength-based, and culturally responsive support.
• Maintain regular contact with families and youth through phone, virtual, community, and face-to-face interactions.
• Meet with youth and families in their homes and communities to understand needs, strengths and goals.
• Coordinate services and supports addressing developmental, emotional, physical, and functional needs.
• Develop and implement individualized, family-centered service plans based on youth needs.
• Connect families with formal and informal supports, providers, schools, and other agencies.
• Promote meaningful inclusion in the community and home environment.
• Coordinate benefits and services to ensure appropriate supports regardless of funding source.
• Authorize eligible supports and services through the CLTS Program.
• Provide transition support when youth enroll or transition out of CLTS.
