Job summary

Lund Co.’s Regional Partnership Program seeks a dedicated Case Manager to support families, screen for substance use, and link clients to treatment across Barre and Hartford DCF offices in Vermont. The role coordinates referrals, addresses barriers like transportation and childcare, and liaises with DCF and providers to support engagement in services across homes, community spaces, and offices.

Qualifications

Minimum of an associate degree required; bachelor’s preferred.
Experience providing case management in substance use, recovery, or mental health settings is preferred.
AAP certificate or ability to test for certification within first year; Lund provides support.
Experience working with families, in multidisciplinary teams, and knowledge of community resources is preferred.
Maintain professional boundaries and client confidentiality per state/federal regulations.
Proficient with technology: Microsoft Suite, EHRs, and video conferencing (Zoom/Teams).
Valid VT driver’s license and reliable transportation; occasional statewide travel may be required.

Responsibilities

Conduct substance use screenings and coordinate referrals to assessment and treatment providers.
Link families to appropriate services and reduce barriers to engagement (transportation, childcare, stigma).
Provide consultation and training to DCF staff on substance use and treatment services.
Liaise between the treatment system and DCF Family Services across various settings (offices, community spaces, homes).
