Job summary

Lund’s Regional Partnership Program is seeking a dedicated Case Manager to support families and address substance use challenges across Vermont communities. The role will work closely with DCF Family Services, screening for issues, linking to treatment, and removing barriers to engagement.

The starting rate is $25/hour, with duties conducted at DCF offices and in community settings. Candidates with an associate degree and relevant experience are encouraged to apply.

Qualifications

• Associate degree required; bachelor's in social work or related field preferred.
• Experience providing case management in substance use, recovery, or mental health settings preferred.
• AAP certificate or ability to test for certification within first year; Lund provides support.

Responsibilities

• Screen for substance use and coordinate referrals to assessment and treatment.
• Engage families and address transportation, childcare, and stigma barriers.
• Collaborate with DCF staff and treatment providers; provide liaison between treatment system and DCF Family Services.
• Deliver services in DCF offices, community spaces, and caregivers’ homes to meet family needs.
