Job summary

Qualifications

• Bachelor's degree in Sociology, Psychology, Social Work, or Human Services.
• Two years of related experience.
• Ability to express self, in English, clearly in writing and speech.

Responsibilities

• Conduct outreach to identify individuals and families who would benefit from services.
• Maintain accurate documentation of services provided.
• Motivate clients to change behavior using Motivational Interviewing and Stages of Change.
• Encourage retention and family involvement to improve outcomes.
• Conduct home visits and coordinate care with families and providers.
