Community Care Network is seeking a Care Manager to join our team in Vermont. The role manages a caseload, coordinates services, provides supportive counseling, and collaborates with the treatment team to ensure quality, efficient services.

Qualifications

• Associates degree preferred; 3 years working in a related field required.
• Knowledge of community services, primary health care, and mental health/substance use treatments.
• Experience with assessment tools, treatment planning processes, and documenting case management activities.

Responsibilities

• Facilitate and support regular treatment planning, screenings, and reassessments.
• Accept new cases after intake completion and coordinate services with clients.
• Build therapeutic rapport, document plans and service contacts timely.
• Meet with supervisor regularly to review case management and administrative tasks.
• Provide staff training and participate in community education activities.
