Job summary

Lund Co is seeking a dedicated Case Manager to support families and address substance use challenges in Vermont communities. The role collaborates with DCF staff to screen, link to treatment, and remove barriers to engagement for families affected by parental substance misuse.

Responsibilities include screenings, referrals, and training for DCF staff, with services delivered in offices, community spaces, and caregivers’ homes. The starting rate is $25/hour.

Qualifications

• Minimum of an associate degree required with preferred bachelor’s in social work, counseling, or a human services field.
• Experience providing case management in substance use, recovery, or mental health settings preferred.
• Ability to obtain AAP certificate within first year, with Lund support for training.
• Experience working with families and knowledge of community resources preferred.
• Maintain client confidentiality per state and federal regulations.
• Proficient with Microsoft Suite, EHRs, and video conferencing tools.

Responsibilities

• Screen for substance use and coordinate referrals to assessment and treatment providers.
• Link families to treatment services and address transportation, childcare, or stigma barriers.
• Provide consultation and training to DCF staff on substance use and services.
• Liaise between treatment system and DCF Family Services; work in offices, community spaces, and homes.
