Job summary

The CLTS Case Worker at Eau Claire County partners with children with disabilities and their families to assess needs, develop individualized service plans, and coordinate supports that promote safety, independence, and community inclusion. The role requires collaboration with providers, schools, and community partners to achieve positive outcomes.

Responsibilities include conducting thorough assessments, preparing time-limited service plans, and maintaining documentation in accordance with CLTS

Qualifications

• Engages families through respectful, client-centered interactions to support engagement and change.
• Coordinates and facilitates team meetings with providers, schools, and natural supports.
• Maintains knowledge of CLTS services and resources and educates families on their rights.
• Follows CLTS Waiver Program requirements and best practices in case management.

Responsibilities

• Assess needs and develop individualized service plans with families.
• Coordinate services across providers, schools, and community partners.
• Monitor service delivery and strive for safety, independence, and community inclusion.
• Maintain accurate records and ensure confidentiality and reporting compliance.
• Advocate for children and families and participate in supervision and collaboration.
