Job summary

CASES in The Bronx is seeking a FACT Team Leader to direct and supervise the Forensic Assertive Community Treatment team delivering mobile, community-based mental health services for adults with serious mental illness. The role leads clinical and administrative operations, coordinating with the FACT psychiatrist for clinical direction.

The position is full-time, with a 9am-5pm schedule from the Bronx location at 424 East 147th Street, 1st Floor.

Qualifications

• Two years of supervisory experience.
• At least three years of experience providing services to adults with serious mental illness.
• Experience in community mental health settings with in-vivo services.
• At least two years of experience on a multidisciplinary team.
• Experience working with individuals involved in the criminal legal system.
• Fluency in Spanish strongly preferred.

Responsibilities

• Oversee clinical and administrative operations of the FACT Team, including meetings, documentation, intakes/discharges, and schedules.
• Ensure trauma-informed, recovery-oriented services and motivational interviewing.
• Provide regular clinical supervision and professional development opportunities.
• Provide direct clinical care in the community when needed.
• Meet performance outcomes and regulatory standards set by OMH and DOHMH.
• Oversee incident reporting and HIPAA compliance.
• Coordinate monthly Medicaid billing and ensure targets are met.
• Perform other duties as required by ACT Leadership.
