Job summary

New Horizons of The Treasure Coast, Inc. seeks a Team Leader for its Florida Assertive Community Treatment Team (FACT). You will supervise the ITT, coordinate treatment plans, and provide supportive therapy to adults with severe mental illness, while advocating for member rights and community integration.

The role emphasizes daily living support, problem solving, and collaboration with families, courts, and healthcare partners in delivering comprehensive, community-based care.

Qualifications

• Master's degree in behavioral science required.
• Experience with adults with severe and persistent mental illnesses or similar needs.
• Valid Florida Driver's License with 6 points or less in 3 years.
• Pass Level 2 Background through DCF and AHCA Clearinghouse.

Responsibilities

• Provides case management for an assigned group of members and coordinates and monitors the ITT.
• Develops, writes, implements, evaluates, and revises overall treatment goals and plans with the ITT.
• Provides individual supportive therapy and symptom management.
• Ensures immediate changes are made in treatment plans as members' needs change.
• Educates and supports members' families and advocates for members' rights.
• Assesses members' mental illness symptoms and response to treatment and adjusts plans as needed.
• Provides vocational-supportive counseling to identify goals and work-related skills.
• Assists with activities of daily living through problem solving, side-by-side services, and supervision.
• Offers social-skill development and assertiveness training to increase community participation.
• Supports advocacy and coordination for housing, entitlements, and services; assists with daily living needs.
• Assists the Team Leader in day-to-day administrative operations of the FACT team.
