Job summary

Northeast Counseling Services is seeking a dedicated Mental Health Professional to join the Forensic Assertive Community Treatment (FACT) Team. The role focuses on delivering evidence-based counseling and behavioral health services to individuals with serious mental illness in the community, collaborating with a multidisciplinary team to support recovery and stability.

Qualifications

• Master's Degree in Social Work, Psychology, Counseling, Rehabilitation Counseling, or a related behavioral health field from an accredited college or university.
• Valid Pennsylvania Driver's License.

Responsibilities

• Provide individual counseling and behavioral health interventions in community settings.
• Develop and implement person-centered treatment plans.
• Assist individuals in achieving recovery and wellness goals.
• Provide crisis intervention and support as needed.
• Collaborate with a multidisciplinary treatment team.
• Maintain timely and accurate documentation.
• Coordinate services with community providers and resources.
• Promote independence, stability, and community integration.
