Job summary

The Center for Alternative Sentencing and Employment Services (CASES) is seeking a FACT Team Leader in Bronx, NY. The role oversees clinical and administrative operations of a multidisciplinary FACT team delivering evidence-based, community-based mental health treatment to adults with serious mental illness and criminal justice involvement.

Full-time, Monday–Friday, 9am–5pm, hybrid work option with up to one day remote meetings.

Qualifications

• LCSW, LMSW, LMHC, RN, or Licensed Psychologist (PsyD/PhD) license required.
• Two years of supervisory experience.
• At least three years of experience providing services to adults with serious mental illness.
• Experience in community mental health settings that provide services in the community.
• At least two years on a multidisciplinary team.
• Experience working with individuals involved in the criminal legal system (preferred).

Responsibilities

• Oversees the clinical and administrative operations of the FACT Team, including daily team meetings, clinical meetings, and documentation.
• Ensures culturally responsive, trauma-informed, recovery-oriented services with motivational interviewing.
• Provides regular clinical supervision, training and professional development for team members.
• Provides direct clinical care in the community when needed.
• Ensures the team meets OMH licensure and NYC DOHMH contracting standards.
• Oversees incident reporting and compliance with IRC policies.
• Collaborates with QA to ensure compliance with OMH and HIPAA regulations.
• Advises senior clinical leadership on high-risk cases and staffing issues.
• Coordinates monthly Medicaid billing using the EMR and tracks billing targets.
• Performs other duties as required by ACT Leadership.

FACT Team Leader

Job Summary

The Forensic Assertive Community Treatment (FACT) team is a multi-disciplinary team that provides evidence-based, mobile, community-based mental health treatment to promote the recovery and community integration of individuals age 18 and up with serious mental illnesses, recent history of criminal legal involvement, high rates of co-occurring substance use disorders and high utilization of emergency/crisis services. The FACT team serves consumers on parole, probation, returning to the community from jails and prisons, participating in treatment court services, and those with histories of high-risk and violent behavior.

Salary

The salary for the FACT Team Leader is $101,458 - 104,499.

Shift Hours

The FACT Team Leader is full-time Monday through Friday 9am-5pm shift.

Workplace Flexibility

Flexible - (for roles that have days that can be partially remote, e.g., being able to draft notes at home after going to the office/community in the morning). Maximum 1 day per week virtual team meeting

What you will be doing

• Oversees the clinical and administrative operations of the FACT Team, including but not limited to: daily team meetings, regular clinical meetings, timeliness and quality of clinical documentations and reporting, intakes and discharges, team’s after-hour emergency line, use of service dollars, and team members’ schedules
• Ensures the team delivers culturally and developmentally appropriate, trauma-informed, recovery-oriented services and effectively applies motivational interviewing
• Provides regular clinical supervision, training and professional development opportunities for team members
• Provides direct clinical care and services to consumers in the community when needed
• Ensures the team meets its performance outcomes and operational standards as defined by New York State Office of Mental Health (OMH) licensing standards and New York City Department of Health and Mental Hygiene (DOHMH) contracting standards
• Oversees team’s incident reporting procedures according to OMH standards and ensure team adheres to the Incident Review Committee (IRC) policies and procedures
• In collaboration with the Quality Assurance department, conducts utilization review and quality assurance reviews to ensure that the FACT team operates in compliance with New York State OMH guidelines and regulations, New York City DOHMH operating standards, as well as HIPAA confidentiality regulations
• Consults with senior clinical leadership on complex, high-risk cases, as well as team and personnel issues
• Coordinates monthly Medicaid billing using electronic medical record to select billable progress notes and ensure team achieves billing targets defined by program budget
• Performs other duties as required by ACT Leadership and executive staff

What we are looking for

• LCSW, LMSW, LMHC, RN, or Licensed Psychologist (PsyD/PhD)
• Two years of supervisory experience
• At least three years of experience providing services to adults with serious mental illness
• Experience working in community mental health settings that provides services to recipients in-vivo in the community
• At least two years of experience working on a multidisciplinary team
• Experience working with and knowledge of the needs and challenges faced by individuals involved in the criminal legal system
