Pathways Vermont is seeking a FACT Program Manager to lead the Forensic Assertive Community Treatment team in Chittenden County, Burlington, VT. The role focuses on reducing homelessness and improving health, safety, and community integration for Vermonters under DOC supervision.

The Program Manager provides leadership, manages staff performance, ensures program fidelity to the FACT model, and collaborates with DOC, DMH, Housing First, funders, and partners to deliver high-quality services.

• Oversee day-to-day program operations of the FACT team and relationships with clients and partners.
• Ensure quality, effectiveness, consistency, and sustainability of the FACT program.
• Develop, implement, and evaluate annual program goals and performance measures.
• Manage the FACT grant and collaborate on grant reports and partner meetings.
• Ensure compliance with grant, contract, Medicaid, and documentation requirements.
• Monitor the FACT program budget with leadership and identify variances.
