Job summary

Pathways Vermont is seeking a FACT Program Manager to lead the Forensic Assertive Community Treatment team in Chittenden County, based in Burlington, VT. The role oversees operations, staff supervision, partnerships, and program fidelity to the FACT model, while meeting grant and contract requirements.

Responsibilities include budgeting, reporting, staffing, and collaborating with DOC/DMH and community partners to reduce homelessness and improve health outcomes for participants; full-time

Qualifications

• Supervisory experience in human services is preferred.
• Strong written and verbal communication skills are essential.
• Ability to network with providers, funders, and community stakeholders.
• Valid driver’s license and reliable vehicle required.

Responsibilities

• Oversee day-to-day operations of the FACT team.
• Ensure program quality, outcomes, and fidelity to the FACT model.
• Develop annual goals, priorities, and performance measures.
• Manage grant and budget in collaboration with finance and leadership.
• Hire, train, and supervise FACT staff.
• Maintain documentation and ensure compliance with contracts and regulations.
