Job summary

The Riverside Premier Rehabilitation & Healing Center is seeking an experienced Physical Therapist (PT) to join our facility in New York. The PT will assess patients requiring rehabilitation, design and administer treatment plans, and educate patients and families under physician direction.

The ideal candidate will have an accredited PT degree, an active license, and experience with long-term or geriatric care.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Must hold a valid professional license and registration for state of practice.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred.
• Strong documentation skills and ability to write evaluations and notes.
• Experience with case mix index and PPS scheduling is a plus.

Responsibilities

• Assess referred patients requiring rehabilitation and develop treatment plans.
• Provide direct patient care and establish goals based on patient capacity.
• Document patient condition and progress; educate patients and families.
• Supervise Physical Therapy Assistants and ensure quality care.
• Collaborate with other disciplines to deliver patient-centered care.
• Complete discharge documentation and ongoing progress notes.
• Adhere to confidentiality, HIPAA, and residents' rights.
