Job summary

The Riverside Premier Rehabilitation & Healing Center is seeking an Experienced Physical Therapist (PT) for our facility. The PT will assess referred patients, develop and implement treatment programs, and document patient progress under physician direction.

Collaboration with all disciplines and supervision of PT assistants are essential. Candidates should have graduated from an accredited PT program, hold a valid state license, and have strong documentation and patient service skills.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and professionalism.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals aligned to patient capacity and tolerance.
• Document patient condition and educate patients and families in physical therapy methods.
• Report to work on time and adhere to scheduled hours.
• Perform other duties as assigned by Director of Rehabilitation.
