Job summary

Qualifications

• Graduated from an accredited Physical Therapy program.
• Valid professional license and registration for state of practice.
• Experience supervising Physical Therapy Assistants.
• Long-term/geriatric care experience preferred.
• Strong documentation skills and timely charting.
• Performs evaluations, progress notes, recertifications and discharge summaries.
• Collaborates with all disciplines to provide patient-centered care.
• Demonstrates exceptional patient service and clinical excellence.
• Punctual, professional, and adheres to schedule.

Responsibilities

• Assess referred patients and develop treatment plans.
• Provide direct patient care and set goals according to patient capacity and physician direction.
• Educate patients and families on physical therapy methods.
• Document patient condition and progress; maintain records.
• Oversee and supervise Physical Therapy Assistants.
• Coordinate with disciplines to optimize patient outcomes.
• Ensure confidentiality and HIPAA compliance.
• Perform other duties as assigned by the Director of Rehabilitation.
