Job summary

The Riverside Premier Rehabilitation & Healing Center is seeking an Experienced Physical Therapist (PT) for our facility in New York. The PT will assess patients requiring rehabilitation, develop and administer individualized treatment plans, and document progress and outcomes.

The role involves supervising Physical Therapy Assistants and collaborating with other disciplines to ensure high-quality patient-centered care.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing.
• Perform evaluations, weekly progress notes, recertifications and discharge documentation.
• Collaborate with all disciplines for patient-centered care.
• Demonstrate exceptional patient service and clinical excellence.
• Punctual, professional, and adherent to schedule.
• Perform other duties as directed by the Director of Rehabilitation.
• Comply with confidentiality, HIPAA and Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred.

Responsibilities

• Assess referred patients and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity and tolerance.
• Document patient condition and educate patients and families in physical therapy methods.
