Job summary

Responsibilities

• Lead enterprise-wide Case Management centralization and standardization across all facilities.
• Oversee policy development, CMS compliance, and performance metrics.
• Partner with CNOs, CMOs, physician advisors, and post-acute partners to optimize care transitions.
• Drive strategic initiatives to reduce readmissions and improve patient throughput.
• Develop and manage enterprise budgets and technology investments.
