Job summary

Ebenezer is seeking a Registered Nurse (RN) to ensure the safety and health needs of adult long-term care and transitional care unit patients in Minneapolis, MN. You will work with a team of CNAs, therapy, and other disciplines to support residents.

The position includes evenings, 64 hours every two weeks, and every other weekend/holiday rotation. Responsibilities include monitoring care plans, coordinating medications, assessing goals, and documenting care and changes.

Qualifications

• Active MN RN license required or by start date.
• BLS certification required upon hire.
• Experience with electronic health records preferred.
• Comfortable with evening shift and hospital/long-term care settings.

Responsibilities

• Monitor and document personal cares per individual care plans.
• Coordinate medications and treatments per physician orders and facility policies.
• Assess care plans and resident goals; revise as needed.
• Provide timely reports on incidents and changes in status.
