Qualifications

• Pharmacist licensure in Ohio is required.
• Completion of an ASHP residency or equivalent practice experience required.
• Pharmacy Board Certifications or other certifications preferred.
• Advanced-level degree in healthcare field preferred.
• Collaborative practice experience required.
• ACLS certification may be required to obtain per manager's determination.
• Pharmacokinetic dosing experience and drug therapy management experience expected.

Responsibilities

• Dispense and compound medications and counsel patients on medications.
• Interpret prescriptions and manage patient drug therapy in collaboration with providers.
• Precept pharmacy students and residents; mentor staff and learners.
• Provide drug information to healthcare professionals and patients as needed.
• Participate in ambulatory care, retail, and medication safety initiatives.
• Ensure safe, cost-effective, and evidence-based medication use.

Education

Bachelor of Science or Doctor of Pharmacy degree

Current Ohio pharmacist license

Basic Life Support Certification

ASHP accredited residency

Pharmacy Board Certifications

Advanced-level degree in healthcare

Collaborative practice experience

Advanced Cardiac Life Support Certification

Pharmacokinetic drug dosing experience

Drug therapy management
