Responsibilities

• Implement Person-Centered Plans, behavior plans, and nursing care plans.
• Provide supervision, personal care, and life skills training.
• Assist with meal preparation, household upkeep, and community activities.
• Document daily progress, incidents, and health data in agency systems.
• Transport residents to appointments and activities.
• Respond to crises and communicate concerns to medical and supervisory staff.
• Maintain a positive, respectful, and safe home environment.
