Job summary

Breaking Ground seeks a Clinical Coordinator to supervise Case Managers and implement Harm Reduction and Housing First strategies to deliver high-quality care for clients.

The role requires NYS LMSW licensure and at least 3 years of relevant experience, with supervisory responsibilities. You will work during business hours with on-call availability for emergencies and special projects, collaborating with outreach teams to support housing placement and client progress.

Qualifications

• NYS Licensed Masters Social Worker (LMSW) required.
• 3+ years of experience with related populations; supervisory and administrative experience preferred.
• Clinical understanding of Housing First, client-centered motivational interviewing, Harm Reduction models, behavioral and medical health systems.

Responsibilities

• Supervise a team of Case Managers.
• Provide regular, structured supervision meetings for direct reports.
• Promote strategies for case management using Housing First, Harm Reduction, CPI, motivational interviewing and trauma-informed care.
• Develop and implement individual service plans; ensure documentation of client contact and progress is comprehensive and timely.
• Interview and assess potential clients.
• Provide clinical direction and leadership to the program, troubleshoot and strategize around client engagement and challenging behaviors.
• Promote housing placement as a success metric and ensure clients are ready for housing placement.
• Oversee development and implementation of service plans and documentation.
• Assess and evaluate client functioning.
• Supervise and conduct case conferences.
• Coordinate care with multiple service providers, especially outreach teams.
• Work with outreach teams to help clients succeed in transitional housing.
• Support and guide front line staff in de-escalating conflicts.
• Perform other duties as assigned.
