Job summary

Advocate Health is seeking a Speech-Language Pathologist for Outpatient Rehabilitation in Crystal Lake. The role involves evaluating patients with diverse speech, language, voice, cognitive, and swallowing disorders and developing a written plan of care with targeted treatment.

You will monitor progress, adjust plans as needed, and communicate with physicians and staff to ensure optimal care. A Master’s in Speech Pathology and CCC SLP licensure are required, with BLS obtainable within 90 days.

Qualifications

• Master’s degree in Speech Pathology required.
• CCC SLP licensure required (ASHA).
• BLS for Healthcare Providers required or obtained within 90 days.

Responsibilities

• Interprets physician referrals and conducts speech and language evaluations for patients with speech, language, voice, cognitive and swallowing disorders.
• Establishes a written plan of care and implements individualized treatment, monitors outcomes, and updates physicians.
