Job summary

Cedarhurst of St Charles is seeking an experienced LPN or RN for evening coverage (3:00 p.m. – 11:00 p.m.) with rotating weekends. The role delivers and coordinates resident care under physician orders and supports a resident-centered environment in a senior living community.

The ideal candidate has a nursing license, strong clinical skills, and a passion for teamwork. You will administer medications, monitor residents, educate families, and collaborate with the Director of Nursing to maintain

Qualifications

• Graduation from an accredited nursing school and a current state license for LPN or RN is required at all times.
• 1+ years of nursing experience preferred but new graduates are welcome.
• CPR or BLS certification preferred.
• Demonstrated proficiency in common digital tools and applications, including Microsoft Office programs (Outlook, Excel, Word), web-based platforms, electronic health record systems, and any HR-related systems as applicable.

Responsibilities

• Provide nursing services in accordance with nurse practice act and center regulations.
• Direct care staff regarding resident needs in collaboration with the Director of Nursing.
• Review and revise assessments and care plans at admission and as conditions change.
• Transcribe physician orders and assist with examinations as needed.
• Initiate resident education plans per physician guidance and resident needs.
• Observe residents and report condition changes to physicians and proxies; intervene as instructed.
• Administer and document medications following the five rights.
• Assist and monitor residents with activities of daily living and transfers as needed.
