You will administer medications, monitor residents, and document care while collaborating with an interdisciplinary team to ensure quality outcomes for skilled nursing and rehabilitation residents.

Qualifications

• Active Alabama LPN license in good standing.
• Current CPR/BLS certification.
• Experience in skilled nursing, rehabilitation, long-term care, or post-acute care preferred.
• Strong clinical, communication, and teamwork skills.

Responsibilities

• Provide direct nursing care and coordinate resident care during the assigned shift.
• Administer medications and treatments according to physician orders.
• Monitor residents and promptly report changes in condition.
• Supervise and support CNAs on the assigned unit.
• Maintain accurate and timely nursing documentation.
• Communicate with residents, families, physicians, and the interdisciplinary care team.
• Follow infection prevention, safety, and regulatory requirements.
